You are the Medical Director of the OB/GYN department. You have been asked to review a case by one of your doctors. A couple who has been trying for years to have a child and spent all of their life savings was finally able to conceive using in vitro fertilization (IVF). They have been advised by their doctor that this will be the last attempt they can make due to their age. From the process, there are 4 embryos. The doctor has recommended that only 2 be implanted due to the mother’s age and overall health and the other 2 be frozen (although it is unlikely that these will be viable to use if not implanted now). Considering all of this, the patient wants all four implanted. She is fearful that something will go wrong with any of these and wants to have the best chance of a life birth, regardless of the potential negative impact on her health and financial strain if all four go full-term. Draft a memorandum for the provider on how to address the issues associated with this case. Indicate if you think we should do as she asked and implant all four or should we do what is medically advised. How does your decision include the health of mother and embryos? Can we ethically dictate to her how many we can implant knowing that they can’t afford 4 children at one time? Also, outline some suggestions on her non-medical options if there is an issue with the IVF and pregnancy.
