Concept Map Directions

What: Visual representation of your patient utilizing elements of a traditional care plan.
Purpose: To synthesize patient data & apply critical thinking skills

A Concept Map…

• Is a visual/graphic representation of your patient
• Utilizes elements of a traditional care plan and critical thinking to synthesize data and understand relationships between variables
• Allows the nursing student to make & understand connections between disease processes (pathophysiology), disease & symptoms management, nursing process, nursing interventions, and the psychosocial effects disease can have on an individual & family.
• Helps bridge the gap between theory & practice in a new manner.

Directions:

1. Choose 1 disease process on the Concept Map #1 discuss board.
2. You will present how you would expect a patient’s information and your assessment & interventions would present in graphic/ visual form.

3. Your concept map must include the following:
1. Basic Patient Info: age (choose most common age for dx), gender, allergies
2. Medical diagnoses
a. Pathophysiology: include 1-2 sentences
i. Use proper APA formatting for any pathophysiology you include.
b. Medical interventions: Labs, procedures, medications typically seen with your chosen dx
3. Assessment data (vital signs, LOC, skin condition etc.) – what would you expect to see in a patient with your chosen dx?
4. Expected nursing diagnosis & interventions
5. Expected goals/expected outcomes
6. Expected patient Teaching done/planned
7. Evaluation (did your interventions work? Or what criteria would you use to evaluate patient’s improvement if there was not time to see results)

4. Review your Concept Map with your clinical instructor at least 1 week prior to the due date to give you time to make any changes.
