MY INITIAL RESPOND:
Today’s patient care is more complicated than ever, as a result of the rise in the geriatric population that has at least one chronic condition such as kidney disease, diabetes, Alzheimer’s disease, arthritis, etc. The complexity of these illnesses calls for specialized care, which, in turn, underscores the imperative of a multidisciplinary approach in patient treatment. With a multidisciplinary team of professionals working toward the alleviation of patient illness and suffering, there is a need to collaborate and communicate effectively. According to the American Association of Colleges of Nursing (2015), effective communication and collaboration between medical professionals are of the essence in the provision of patient-centered care. The benefits of professional collaboration and communication are that these allow medical teams to lessen patients’ stress levels besides positively impacting care upshots. Also, teamwork helps lower the number of issues linked to nursing burnout. Nurses, as team members, are not the only caregivers; thus, they do not bear the sole responsibility for the health of patients. Nurses need their teammates’ support to make appropriate decisions.
In the course of the BSN program, I have worked towards meeting the provisions of Essential VI, in terms of collaborating and communicating effectively. I understand the significance of interprofessional education in equipping baccalaureate graduates with the baseline competencies and confidence to interact plus communication skills to improve practice and yield better patient upshots. According to Nester (2016), success in today’s health care environments accentuates the essence of interprofessional education and practice. Personally, whenever I have taken part in patient care, I have always ensured that I work in close collaboration with other medical staff by helping other nurses dealing with difficult patients, assisting with activities of daily living, asking for help from my colleagues, and helping them stay on schedule.

Today, delivery of patient care involves communication among multiple healthcare professionals. Collaboration in health care is defined as health care professionals assuming complementary roles and cooperatively working together, sharing responsibility for problem-solving and making decisions to formulate and carry out plans for patient care (Vermeir, Vandijck, Degroote, Peleman, Verhaeghe, Mortier, Hallaert, Van Daele, Buylaert, & Vogelaers, 2015). Effective clinical practice thus needs accurate communication of critical information. Lack of communication creates situations where medical errors can occur. These errors have the potential to cause severe injury or unexpected patient death.
Essential IV was fulfilled in clinicals. During report, you can see how giving good report increases quality of care, as well as, not giving good report can lead to delay in care. For example, during one of my clinicals, one of my nurse’s patients had a scheduled ORIF early in the morning. The night shift failed to mention that she didn’t finish the checklist for the patient’s surgery. So when transport came to take the patient to the OR, my nurse was scrambling to get the checklist finished. It delayed the patient’s surgery unnecessarily.

Why are communication and collaboration among healthcare professionals critical to delivering high-quality and safe patient care?
Communication and collaboration are two important integral aspect of a profitable safe and high quality patient care. As stated by Merlino (2017), “Communication is the cornerstone of healthcare.” It is important that effective communication is established between every person involved in patient care to ensure that the safety of the patient is secured at the very beginning, during, and later stages of provision of healthcare services. When communication between health care providers is inadequate, the safety of the patient is at risk for several reasons such as; lack of critical information, misinterpretation of information, unclear orders over the telephone, and overlooked changes in status. (O’Daniel and Rosenstein, 2008). These reasons pose uncertainties between everyone involved in the provision of health care and causes danger to the patient. O’Daniels an Rosenstein (2008) further articulates that, “Effective communication among staff encourages effective teamwork and promotes continuity and clarity within the patient care team.”
In health care, collaboration is described as professionals working hand in hand cooperatively, complementing their specified roles, solving problems and decision making to carry out plans for patient care. (O’Daniels an Rosenstein, 2008). Rendering healthcare services requires interdisciplinary teamwork by establishing the role of everyone and carrying out their responsibilities effectively. In the article published by HRH Global Resource Center (n.d.), has identified the following three reasons why teamwork became an important health intervention; (1) Clinical care is becoming more complex and specialized, (2) Researches have found that working together reduces the number of medical errors and increases patient safety, and (3) Because teamwork is centered on solid communication, patient and their families feel more at ease, accept treatment, and feel more satisfied with their health care.
How you met Essential VI during the BSN program. Provide specific examples.
There are numerous instances that Essential VI is highly emphasized during my BSN program. This is specially highlighted and accentuated now that I am on my preceptorship. During our clinical orientation and simulation, our clinical instructors and assigned nurses in the hospital site, incorporated effective communication techniques, including negotiation and conflict resolution to produce positive working relationship. They have exhibited effective ways of communicating between the nurses, doctors, lab workers and patient. They continuously remind us of teamwork. The team leader has demonstrated an effective strategic communication skill that everyone follows as always. Lastly in agreement with Kerps (2016), He stated that, “Team members should be able to use strategic communication to provide important input on making complex health care decisions, contribute to the delivery of team-based care, and enhance health outcomes.”
