Implementing evidence-based practice (EBP) improves the quality of care. Evidence-based practice is the process of combining external research with internal expertise along with patient desires (Lippincott Nursing Center, 2020). In the specialty of obstetrics, kangaroo care during the “golden hour” is essential. Kangaroo care is implemented in healthcare because the evidence proves it is beneficial for the infant and the mother.
The first hour of an infant’s life is called the golden hour when evidenced-based practices are implemented (Neczypor & Holley, 2017). Kangaroo care is holding the infant with only a diaper on the mother’s bare chest. Studies show that kangaroo care in the golden hour decreases infant mortality, especially for preterm infants. Specifically, kangaroo care decreases the likelihood of hypothermia and hypoglycemia (Neczypor & Holley, 2017). Neczypor and Holley (2019) also mention other practices during the golden hour that improve infant outcomes. Zehra and Rukiye (2020) discuss the effects that kangaroo care has on mother-infant bonding. Immediate skin to skin contact with the infant after birth is essential. Kangaroo care improves the mother’s stress and anxiety levels, improves breastfeeding, stabilizes the infant’s vital signs, and enhances mother-infant bonding. Interestingly, other studies show that mothers in a nuclear family have better maternal-infant bonding than those living in an extended family. Mothers who live in extended family environments have less time for themselves and time with the infant (Zehra & Rukiye, 2020). The Value of Evidence
Kangaroo care is a standard practice in OB nursing due to research and evidence. EPB empowers nurses to care for patients with confidence (Lippincott Nursing Center, 2020). When I place an infant skin to skin with a mother, I know I am making a difference in the health outcomes for both of them. I can educate the parents about kangaroo care and empower them to make informed decisions as well. Research is available to back up the claims on the benefits of kangaroo care. Therefore, I do not have to rely on intuition or experience when educating parents.

Evidence-Based Practice
Introduction
Evidence-based practice is a problem-solving approach to clinical decision making, which utilizes the best scientific evidence available, the clinicians’ experience, and the patient’s preferences (Storey et al., 2019, p. 208). This paper aims to describe an evidence-based practice (EBP) observed at work and the clinical practicum and describe the significance of EBP in Ambulatory care nursing.
48 Hour Discharge Follow-up call
The strategies utilized to avoid preventable adverse events after hospital discharge and prevent rehospitalization include patient engagement in discharge planning, transitions coach, communication with outpatient provider, timely follow-up visits, home visits, and postdischarge telephone calls (Agency for Healthcare Research and Quality, n.d.).
During my practicum and at work, a practice noted is the postdischarge telephonic follow-up call completed by the primary care nurses. The nurses make follow-up calls on every patient discharged from the emergency room or the inpatient units. The nurses review the patient’s health status, medicines, appointments, home services, and actions when problems emerge to support the patient and family during this transition period to prevent readmissions.

Clark & Milner (2013) researched the impact of postdischarge follow-up calls. The authors discovered that follow-up calls after hospital discharge reduced readmissions. Patients who received these calls perceived their hospital visits more positively and scored the hospital with top box scores on HCAPS surveys.

Hamar et al. (2018) conducted a study to evaluate the effect of telephone support after hospital discharge to reduce early hospital readmission. The study showed that the postdischarge follow-up calls reduced readmissions within 28-days of initial release and avoided a substantial amount of readmission cost.
Value of Evidence-Based Practice (EBP)
EBP is significant because researchers have developed and evaluated interventions to enhance patient capacity to prevent and self-manage chronic health conditions (Leppin et al., 2018). Therefore, with the use of EBP, the clinicians integrate research findings into practice and choose effective interventions to improve patient experience, reduce per capita cost of care and improve population health (Lavenberg et al., 2019, p. 4).
Conclusion
EBP is a core competency among healthcare professionals (Chan et al., 2020). It contributes to the best clinical decision-making and outcomes for patients and families as this practice utilizes the best research evidence, expert clinical judgment, and patient preferences and values (Chan et al., 2020).
